Erro na hora de fazer Update e Upgrade [RESOLVIDO]

Olá ! Estou com problemas na hora de fazer o Update e o Upgrade no sistema, se alguém pode me ajudar agradeço.

Quando executo o comando:

# apt-get update
# apt-get upgrade

tenho como reposta no final o seguinte

Lendo listas de pacotes... Erro!
E: Erro de leitura - read (5: Erro de entrada/saída)
E: As listas de pacotes ou os arquivos de estado não puderam ser analisados ou abertos.

Poste saída do comando: lsb_release -a

Poste saída comando:: cat /etc/apt/sources.list

Está parecendo falha no HD !

-vc está usando xubuntu-15.10 que ainda terá suporte até julho/2016
-agora vá em programas-atualizações >> outros programas >> desative todas PPA e tente atualizar novamente
